At Putnam Farm at Danvers, assisted living means peace of mind for residents and their families.

Situated high on a hill,Putnam Farm at Danvers is located on Summer Street adjacent to the Hunt Center. Designed to capture the

residential flavor of the neighborhood, the building's gambrel-style features create the air of a country manor and includes a spacious porch with wonderful views. The
community offers residents and families close proximity to the Geriatric Care Center, shopping, churches, and other community resources.

What is Assisted Living?
Residents in our Traditional Assisted Living Program enjoy the privacy of their own apartment with all the benefits of a full service community. Based on each resident's individual needs, the assisted living program provides residents personalized assistance with activities of daily living such as bathing, dressing, and medication management.

Putnam Farm at Danvers also offers Assisted Living Programs for individuals with memory impairment, featuring The Harbor Program. Harbor means a safe haven which is exactly what our program is all about. It is a secure, specialized assisted living program for those individuals with memory impairment or Alzheimer's disease. The Harbor Day Program is another innovative Putnam Farm at Danvers program developed to assist residents and their loved ones who live with memory impairment.

		The Harbor Program
		The Harbor Program is a safe, active residence for individuals with Alzheimer's Disease and other dementias. Individuals receive only the assistance necessary to remain as independent as possible. The focus of the program is on fostering independence and quality of life within a safe setting, and a home-like environment. Staff assist residents to lead fulfilling active lives, through reminiscence and individual accomplishment. Emphasis is placed on the resident's existing skills and abilities. The Harbor Program has been developed on the belief that participation in any or all tasks is an activity for the individual with a memory impairment. The daily schedule provides needed structure and routine and has the ability to adapt to the individuality of each resident. Encouragement, cueing, and individual assistance is provided by staff, to maximize each resident's potential level of function. 		The Harbor Day Program
		The Harbor Day Program is for those residents on Traditional Assisted Living with memory loss or other cognitive impairments who would benefit from the structure of the daily schedule offered on the Harbor unit. The residents maintain an apartment in Traditional Assisted Living while participating in the Harbor Day Program on the Harbor Unit. The Day Program offers flexibility to couples with differing needs, enabling them to live together in Traditional Assisted Living while one spouse attends the Harbor Day Program. HIGHLIGHTS OF THE HARBOR DAY PROGRAM INCLUDE: Program provided seven days a week Escorts provided back and forth from the resident's apartment to the Harbor Program Residents will be on Harbor unit from breakfast until after dinner which will enable them to participate in all Harbor activities Three meals a day are provided on the unit Hourly wellness checks while on the Harbor unit Self-administered medication management is provided on the unit Personal laundry service Night checks Ongoing assessment of residents needs by Harbor Program Director

		Flexibilty in the Harbor Day Program
		Couples may want to participate in 1/2 day programs in Harbor. Putnam Farm understands that everyone's needs are different and we will customize a plan just right for you and your family. The residents maintain an apartment in Traditional Assisted Living while participating in specialized activity programs in the Harbor Program. The activities are both structured and unstructured and provide meaning, accomplishment, and contentment for the residents in the program. Encouragement, cueing, and individual assistance is provided by the staff to maximize each resident's potential. The goal is to provide structure and routine to the day and activity opportunities that residents may choose from. Halft day program iare sometimes used as a transition to the full Harbor Day Program or for couples who may need respite durning the day.
